Getting started

Here's exactly what happens, so there are no surprises.

Reaching out for psychiatric care takes nerve. We've made the path as clear as we can: who you'll hear from, what you'll fill out, how long the first visit is, and what it costs, before you commit to anything.

The path

From first message to feeling like yourself.

  1. You request an appointment

Click Request an Appointment anywhere on this site, or call (919) 473-6909. Our care coordinator, Xaira or Erika, replies, verifies your insurance benefits and sends your intake paperwork through our secure patient portal. You'll know your expected cost before your first visit.

  2. You complete your intake

In the portal you'll share your history, current medications, pharmacy, and what you're hoping for, and upload a photo of your ID and insurance card. It takes about fifteen minutes. Everything stays inside the secure portal, never in email.

  3. Your first visit: 60 to 90 minutes

By secure video from a private spot in North Carolina, or in person at our Clayton office. Your provider hears the whole story, may order labs, and builds a plan with you. If medication is part of it, prescriptions go to your pharmacy electronically that day.

  4. Follow-up until you're stable

Monthly visits while we fine-tune. Then about every three months once things are steady. You'll have a way to reach us between visits if something feels off.

What to have ready

  • A photo ID and your insurance card (you'll upload these in the portal).
  • A list of current medications and supplements, including doses.
  • The name of your preferred pharmacy.
  • Anything you've tried before and how it went, even roughly.
  • If you track your cycle or your sleep, bring the data. It helps.
  • Questions. Write them down. Visits are long enough to get to them.

If your visit is by video

  • You need to be physically in North Carolina at the time of the visit. That's a licensing rule.
  • Any phone, tablet or computer with a camera works. We send a secure link; there's no app to install.
  • Find a private spot. The car in the driveway counts. So does the nursery during a nap.
  • Headphones help if there are other people in the house.

If your visit is in person

We're at 10027 Clayton Blvd, Suite C, Clayton, NC 27520, Monday through Thursday from 9:30 to 5. Parking is on site. Directions and a map are on the contact page.

Who you'll meet

Your care coordinator handles paperwork, benefits and scheduling and is usually your first point of contact. Your provider is Tiirini Hill, PMHNP-BC, or Sherrie Smith, PMHNP-BC, and you'll see the same provider at every visit. Our billing specialist, Asia, handles claims behind the scenes.

Who we see

Adults 18 and up anywhere in North Carolina. Women's mental health, pregnancy and postpartum are our specialty, and men and partners are welcome too. You don't need a referral.

A note on emergencies. R3 is an outpatient practice and can't provide same-day crisis care. If you're in danger or thinking about ending your life, call or text 988 or go to the nearest emergency room, then let us know so we can follow up.
