Website Privacy Policy

This policy explains how the R3 Women's Wellness, PLLC website (r3womenswellness.com) handles information. It covers the website only. Information you share as a patient, including everything in our secure patient portal, is protected health information governed by our Notice of Privacy Practices and federal and state health privacy law.
What this website collects
Contact form. When you send a message through our contact page we receive the name, email address, phone number (if you provide it), the reason you selected, and your message. The form is delivered to our practice email. We ask you not to include medical details in the form because ordinary email is not a secure channel for health information.
Appointment requests. The "Request an Appointment" button takes you to our secure patient portal, which is operated by SimplePractice under a business associate agreement with us. Information you enter there is protected health information and is covered by our Notice of Privacy Practices and SimplePractice's own security practices, not by this website policy.
Technical information. Like most websites, our hosting provider records basic technical data such as IP address, browser type, pages requested and timestamps in server logs for security and reliability. We do not currently use advertising cookies or sell any information about visitors.
Embedded map. The contact page includes a Google Maps embed. Loading it may send standard technical information to Google under Google's privacy policy.
How we use it
- To respond to your message or question.
- To coordinate scheduling and benefit verification when you ask to become a patient.
- To keep the website secure and working.
We do not sell, rent or share your information for marketing, and we don't send marketing email unless you have asked for it.
Who processes it for us
Our website is hosted by Vercel. Contact form messages are delivered by an email delivery service and arrive in our practice email. Patient intake and scheduling run through SimplePractice. Each of these providers processes information only to provide its service to us.
How long we keep it
Contact form messages are kept in our practice email for as long as needed to respond and for reasonable record-keeping, then deleted. Patient records are retained as required by law and described in the Notice of Privacy Practices.
Your choices
You can contact us by phone instead of using the form. You can ask us to delete a contact form message you sent by emailing the address below. Requests about your patient records are handled under the Notice of Privacy Practices.
Children
This website and our services are for adults 18 and older. We do not knowingly collect information from children.
Changes
If this policy changes, we'll update the effective date at the top of this page.
