Medication management
Start low, go slow, and never on autopilot.
Medication can be the thing that finally lets you feel like yourself again. It works best when it's chosen carefully, started gently, and adjusted by someone who actually knows your story. That's how we do medication management at R3.
How medication management works here
Every medication plan at R3 starts with a full psychiatric evaluation. We don't prescribe to a symptom we haven't understood, and we don't skip the labs that could explain it. Once we agree medication is the right tool, here's what you can expect:
- Start low, go slow. We begin at a gentle dose and increase only as needed. Fewer side effects, more trust in the process.
- Monthly follow-ups until you're stable. We check how you're feeling, what's changed, and whether the dose or the medication needs adjusting. Once things are steady, visits move to about every three months.
- Honest conversations about side effects. You'll know what to watch for, and you'll have a way to reach us if something feels off.
- Integrative support alongside. Sleep, nutrition, supplements and stress all affect how medication works. We address them together, not as an afterthought.
- GeneSight testing when it helps. If you've tried several medications without relief, pharmacogenetic testing can show how your body processes different options and narrow the guesswork.
What we treat with medication
We provide medication management for adults with anxiety, depression, ADHD, obsessive-compulsive symptoms, mood disorders including bipolar spectrum conditions, trauma-related symptoms, PMDD, and insomnia. Our specialty is the perinatal season: planning for pregnancy, pregnancy itself, and postpartum depression and anxiety.
Medication during pregnancy and breastfeeding
This is where so many women get brushed off, told to stop everything or to just "wait until after." It doesn't have to be that way. Many medications have well-studied options for pregnancy and breastfeeding, and untreated depression and anxiety carry real risks of their own. We review the current evidence for your specific medication, weigh both sides honestly, and decide together. When you'd like, we coordinate with your OB or midwife so you aren't caught between two sets of advice.
Our commitment
You'll never be rushed into a prescription, kept on something without a reason, or made to feel like a problem to be managed. You lead your care. We bring the training, the time, and a plan built for your life.
Questions about medication
The worries people bring to their first visit.
Nervous about medication? Worried about dependence, or being judged? These are the questions we hear most.
All FAQsHow do I know if medication is right for me?
You don't have to know before you come in. We look at your history, your symptoms, what you've already tried and your risk factors, then build a plan around you. Sometimes that includes medication. Sometimes it's integrative support, therapy, or treating a medical issue that was driving your symptoms all along.
Will I be on medication forever?
It depends on the diagnosis. Some people take medication for a season and taper off when they're ready. Some conditions are best managed long term. Either way, we reassess together at every visit, and you'll never be left on something without a reason.
Can I stay on my medication while pregnant or breastfeeding?
Often, yes. Many medications have well-studied options for pregnancy and breastfeeding. We review the evidence for your specific medication, weigh the risks of treating against the very real risks of untreated illness, and decide together, usually in coordination with your OB or midwife. Please don't stop anything suddenly on your own.
Do you treat ADHD by telehealth?
Yes. Adult ADHD evaluation and treatment are available by telehealth across North Carolina. Current federal telemedicine rules allow this, and we'll update our process if those rules change.
What if I've tried several medications and nothing worked?
That's exactly when a fresh, thorough evaluation matters most. We'll look for medical contributors that may have been missed, revisit the diagnosis, and consider GeneSight genetic testing to see how your body processes different medications before choosing the next one.
